Working with ADHD Clients as a Solution-Focused Hypnotherapist: Skills, Challenges, and Supervision
ADHD (Attention Deficit Hyperactivity Disorder) presents unique challenges for both clients and therapists. Whether working with children or adults, solution-focused hypnotherapy (SFH) offers a strengths-based approach that empowers individuals to harness their abilities rather than focus solely on difficulties. However, working with ADHD clients requires specific skills, patience, and adaptability. It can also bring frustrations for both the therapist and client, making professional supervision an invaluable resource for reflection and support.

Understanding ADHD from a Solution-Focused Perspective
ADHD is often framed as a deficit, but many individuals with ADHD possess remarkable strengths, including creativity, resilience, and problem-solving skills. A solution-focused approach shifts the narrative from "fixing what’s wrong" to "building on what works". By helping clients recognise their successes and develop strategies that suit their unique thinking patterns, hypnotherapy can be an empowering tool. However, the process isn’t always smooth. ADHD affects attention, impulse control, and emotional regulation, meaning clients may struggle with engagement, consistency, or belief in their ability to change. This can lead to frustrations for both parties.
The Skills Needed to Work Effectively with ADHD Clients
1. Flexibility and Adaptability ADHD clients—both adults and children—may struggle with focus, fidgeting, or impulsivity. A rigid, traditional therapy structure may not work, so the therapist needs to be adaptable. Sessions may require movement breaks, interactive elements, or shorter, more dynamic trance work.2. Clear and Engaging Communication Keeping instructions and discussions simple and engaging is crucial. Using metaphors, stories, or visualisations can help ADHD clients process information more effectively. Asking clear, direct questions also helps them stay on track.3. Patience and Non-Judgement Clients with ADHD often feel like they have "failed" in various aspects of life due to forgetfulness, impulsivity, or difficulties with organisation. A non-judgemental approach that acknowledges their efforts rather than their struggles is key to building trust.4. Strength-Based Focus Instead of emphasising the challenges ADHD brings, solution-focused hypnotherapy encourages clients to recognise their strengths. This might include their ability to think outside the box, hyperfocus on topics they love, or their energy and enthusiasm when engaged in the right task.5. Helping with Routine and Structure One of the biggest challenges for ADHD clients is creating structure and sticking to routines. Hypnotherapy can help clients visualise their ideal day and establish small, achievable habits that work for them. This might include using reminders, alarms, or breaking tasks into manageable steps.6. Managing Frustrations – For Both Client and Therapist ADHD clients may struggle with completing tasks, following through on agreed-upon strategies, or even attending sessions consistently. This can be frustrating for the therapist, who may feel like progress is slow or inconsistent. From the client’s perspective, they may feel frustrated that their brain doesn’t work the way they want it to. They may struggle with feeling overwhelmed, misunderstood, or self-critical. Solution-focused hypnotherapy helps reframe these frustrations by focusing on small wins and reinforcing positive change rather than perceived failures.
Working with Children vs. Adults with ADHD
Children with ADHD May struggle to sit still or follow session structure. Need interactive elements—games, storytelling, or movement. Parents play a crucial role in reinforcing positive changes at home. Hypnotherapy can help with emotional regulation, anxiety, and confidence-building. Adults with ADHD Often have years of frustration from feeling misunderstood or struggling with traditional work and school environments. May benefit from hypnosis for stress reduction, focus, and self-belief. Need practical strategies for structuring their day and managing distractions.
The Importance of Supervision in ADHD Hypnotherapy
Because working with ADHD clients can be unpredictable, therapists benefit greatly from supervision with an experienced professional. Supervision provides:

A space to reflect on challenging cases without frustration affecting the therapeutic relationship.

Practical strategies for managing session flow when a client is highly distracted.

Support in managing therapist frustration, ensuring sessions remain productive and positive.

Fresh insights from a more experienced perspective to fine-tune approaches. A good supervisor helps solution focused hypnotherapists stay grounded, maintain their own well-being, and continue growing in their work.

Final Thoughts
Working with ADHD clients in solution focused hypnotherapy is rewarding but requires patience, flexibility, and a strengths-based mindset. By focusing on what works, helping clients build structure in a way that suits their unique brains, and managing frustrations with empathy, hypnotherapists can create a positive impact. Regular supervision ensures therapists remain supported and equipped to handle the unique challenges ADHD presents.

Solution Focused Hypnotic Gastric Band CPD & Training

As hypnotherapists, we may encounter clients who seek our services while managing complex mental health conditions, including psychosis. However, there are critical considerations regarding duty of care and safeguarding when working with individuals in such states. Why We Wouldn’t See a Client in a Florid State of Psychosis A client in a florid state of psychosis is experiencing an acute phase of symptoms, including hallucinations, delusions, disorganised thoughts, and possibly erratic behaviour. This can impair their ability to differentiate between reality and imagination, potentially heightening their vulnerability and reducing their capacity to engage safely and effectively in hypnotherapy.Example of Identifying Florid Psychosis If a client arrives visibly distressed, speaking about seeing or hearing things that aren’t present (e.g., describing a "voice in the room" or a person others cannot see), this may indicate they are in a florid psychotic state. Other signs might include confused speech, paranoia, or a lack of coherent thought. Observing any of these symptoms is a cue to refrain from providing hypnosis and to direct the client to appropriate mental health services immediately.Importance of Knowing a Client’s Antipsychotic Medication As hypnotherapists, we are not trained in pharmacology, but understanding whether a client is taking antipsychotic medication is essential for safeguarding and tailoring appropriate support. Antipsychotic medications, such as chlorpromazine or risperidone, are prescribed to manage symptoms of psychotic disorders, including schizophrenia and bipolar disorder.

Knowing a client’s medication regimen enables us to identify any contraindications:Hypnosis may interact with medications by potentially amplifying or mitigating effects such as sedation, anxiety, or emotional response. Recognising this can help avoid practices that could unintentionally exacerbate a client’s symptoms. Assess psychological stability: Antipsychotic medications often indicate a more severe mental health condition that requires careful, stable management. For clients on these medications, our awareness helps us remain vigilant to changes in mental health and better prepared to pause sessions if signs of deterioration emerge. Support appropriate referrals: If a client presents symptoms beyond our scope, knowing they are on antipsychotics supports a more informed and prompt referral to mental health services or emergency care. Our Duty of Care and Role Limits It is crucial to remember that we are not trained or qualified to assess or advise on medication. We should make it clear to clients that while we can ask about medication for safeguarding purposes, we will always refer them to medical professionals for guidance about their prescriptions. If you have any concerns or questions about a client’s presentation or their suitability for hypnotherapy, it is essential to consult your supervisor. Your supervisor can help you navigate these situations responsibly and in alignment with best practices for safeguarding.
A Solution Focused Hypnotherapist’s Perspective
When clients come to me for help with weight management, I understand that it’s never just about the weight. It’s about the whole person their habits, mindset, emotions, stress levels, and often, their sleep quality. Weight can sometimes be a symptom of deeper issues, and that’s where solution-focused hypnotherapy can offer a truly holistic approach. Understanding Habits and the Brain’s Role One of the key areas I focus on when working with clients is the way habits are formed and stored in the brain. Often, people develop eating habits that go beyond the physical need for food. Emotional triggers, such as stress or anxiety, can lead someone to seek comfort in food. This pattern of behaviour can become so automatic that it feels impossible to change. This is where the hippocampus comes in. The hippocampus, which is responsible for memory and habit formation, stores these eating habits over time. For example, if a client has always reached for sugary snacks when they feel stressed, this behaviour becomes ingrained and stored as a response pattern in the brain. However, it’s important to remember that while these habits are stored, they aren’t fixed. The brain has the ability to change through neuroplasticity, meaning that with the right approach, new, healthier habits can be formed. In our sessions, I work with clients to address these patterns, helping them recognise when they’re acting out of habit rather than hunger. By using solution focused hypnotherapy techniques, we can access the subconscious mind, where many of these automatic behaviours are stored, and begin to create new, healthier responses.

Habits are formed and stored in the brain

It’s Not Just About the Weight One of the first things I discuss with my clients is that weight management is about much more than just the numbers on the scale. Many clients come in feeling frustrated, having tried various diets and exercise programs without lasting success. Often, that’s because these approaches focus on the symptoms (the weight itself) rather than the underlying causes, such as: • Stress: Stress plays a significant role in weight gain for many people. When we’re stressed, our bodies produce more cortisol, which can lead to weight gain, particularly around the abdomen. Not only that, but stress often triggers emotional eating. Hypnotherapy helps clients manage their stress in a healthy way, reducing the likelihood of turning to food for comfort. • Sleep Issues: Poor sleep is another common issue that can affect weight management. When we don’t get enough sleep, our bodies produce more ghrelin (a hormone that increases hunger) and less leptin (which signals fullness). As part of our work together, I often focus on helping clients improve their sleep patterns, which can have a significant impact on their overall health and well-being. • Self-Esteem and Body Image: Weight issues can also be tied to negative self-image or low self-esteem. Many clients have spent years feeling uncomfortable in their bodies, which can lead to feelings of guilt, shame, or frustration. In our sessions, I help clients reframe how they see themselves, building a more positive relationship with their body and fostering a sense of self-compassion. The Miracle Question: Keeping It Realistic A key part of my solution-focused approach is using the Miracle Question (MQ). This technique helps clients visualise what their future could look like if they achieved their goals. When it comes to weight management, it’s important that this vision is both realistic and meaningful for the client.Rather than focusing on an ideal weight, I might ask: “If a miracle happened overnight and you woke up feeling healthier and more in control of your lifestyle, what’s the first thing you’d notice?” This shifts the focus from a specific number on the scale to the broader benefits of living a healthy life. It could be that the client feels more energetic, more confident, or sleeps better. By keeping the MQ focused on the client’s overall wellbeing, we can work toward practical and achievable goals, rather than setting unrealistic expectations that could lead to disappointment.

MQ to be realistic and meaningful for the client

Defining a Healthy Lifestyle: What Does It Mean for You? One of the most important things to remember is that every client’s idea of a healthy lifestyle is different. As the expert in their own life, the client knows best what changes they want to make and how they want to measure their progress. For some clients, a healthy lifestyle might mean becoming more physically active, choosing nutritious foods, or reducing portion sizes. For others, it might involve addressing emotional eating, reducing stress, or improving sleep. My role is not to dictate what “healthy” should look like for them, but to guide them in defining it for themselves. I often ask questions like: • “When you imagine yourself feeling your best, what kinds of habits do you see yourself having?” • “What small changes would make the biggest difference for you?” • “How would your life look different if you felt more in control of your health?” These kinds of questions allow the client to visualise their version of success. They help me tailor my approach to each individual, ensuring that the changes we work on together are both meaningful and sustainable. The Client is Always the Expert. In solution-focused hypnotherapy, the client is always the expert in their own life. When it comes to weight management, this principle is especially important. No two clients will approach their journey in the same way, and it’s crucial that I respect each person’s individual preferences and pace. I like to explore the client’s previous experiences with weight management. By asking “What’s worked for you in the past?”, I encourage them to reflect on their strengths and build on those. It’s also important to ask “What do you feel ready to start with?”, which helps the client take ownership of their journey and feel empowered to make changes that feel achievable. Another essential aspect of this work is understanding that success isn’t always about the numbers on the scale. While some clients may use weight as a marker of progress, others may focus on how their clothes fit, their energy levels, or how they feel mentally and emotionally. I help clients identify these alternative measures of success, so they can celebrate the small victories along the way. It is key to remember here too that the human body is only designed to lose 2lbs of excess weight a week, so it is imperative that all goals are small and manageable. Even a 1lb loss weekly, leads to a Stone overall loss in 3 months.

The client is always the expert in their own life

Conclusion Working with clients on weight management requires a holistic approach that looks beyond the surface-level issue of weight. By addressing the deeper factors at play, such as stress, sleep, and self-esteem, we can create lasting change that goes far beyond the scale. Using solution focused hypnotherapy, I empower clients to take control of their health in a way that feels right for them. Together, we work to uncover their unique vision of a healthy lifestyle, helping them achieve their goals in a realistic and sustainable way. John Lowson (J Lowson Hypnotherapy) has worked for many years with clients for weight loss and weight gain and created long term changes for many clients. John Lowson is a clinical hypnotherapist and psychotherapist, hypnotherapy lecturer and clinical hypnotherapy supervisor based at the Halifax Hypnotherapy Clinic Ltd 12 Wade House Road, Halifax HX3 7PB Tel - 01422 861989

Working with Weight Management Clients

Personality disorders are complex mental health conditions that can deeply affect a person's life and relationships. People with personality disorders often struggle to maintain stable relationships, and their thoughts, behaviors, and emotions tend to deviate from societal expectations. Understanding these disorders is essential for anyone working in mental health, including hypnotherapists, who may encounter clients with these challenges. In this blog, we will explore what personality disorders are, the different types, their causes, problems associated with them, and how hypnotherapists should approach working with individuals who have personality disorders.1. What are Personality Disorders? Personality disorders are long-term patterns of behavior, thoughts, and emotions that differ significantly from cultural expectations. These patterns typically begin in adolescence or early adulthood and persist over time, affecting various aspects of an individual’s life, including their relationships, work, and social functioning. The key feature of personality disorders is that these patterns of thinking and behaving are inflexible and pervasive, meaning that the individual finds it difficult to adapt to different situations. This can lead to ongoing challenges in how they perceive themselves and others, manage their emotions, and interact with the world around them.Personality disorders are categorized into three clusters by the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), based on similarities in symptoms:Cluster A: Odd or eccentric behavior (e.g., paranoid, schizoid, schizotypal personality disorders). Cluster B: Dramatic, emotional, or erratic behavior (e.g., borderline, narcissistic, antisocial, and histrionic personality disorders). Cluster C: Anxious or fearful behavior (e.g., avoidant, dependent, obsessive-compulsive personality disorders).

The three clusters of personality disorders
2. What are the Different Types of Personality Disorders and How Are They Diagnosed? There are 10 recognized types of personality disorders, which fall into the three aforementioned clusters: Cluster A (Odd or Eccentric Disorders) Paranoid Personality Disorder: Characterized by pervasive distrust and suspicion of others, often interpreting their motives as malevolent. Schizoid Personality Disorder: A pattern of detachment from social relationships and a limited range of emotional expression. Schizotypal Personality Disorder: Marked by acute discomfort in close relationships, along with distorted thinking and eccentric behavior. Cluster B (Dramatic, Emotional, or Erratic Disorders) Antisocial Personality Disorder: Involves a disregard for the rights of others, impulsivity, and a lack of remorse for harmful actions. Borderline Personality Disorder (BPD): Characterized by unstable relationships, self-image, and emotions, with a strong fear of abandonment and a pattern of impulsive behaviors. Histrionic Personality Disorder: Excessive emotionality and a need for attention, often manifesting in dramatic or theatrical behavior. Narcissistic Personality Disorder: A pattern of grandiosity, need for admiration, and lack of empathy, often accompanied by feelings of entitlement. Cluster C (Anxious or Fearful Disorders) Avoidant Personality Disorder: Characterized by social inhibition, feelings of inadequacy, and hypersensitivity to criticism or rejection. Dependent Personality Disorder: A pervasive need to be cared for, leading to submissive and clinging behaviors and a fear of separation. Obsessive-Compulsive Personality Disorder (OCPD): A preoccupation with orderliness, perfectionism, and control at the expense of flexibility and openness. How Are Personality Disorders Diagnosed? Diagnosing a personality disorder requires a comprehensive psychological evaluation conducted by a mental health professional, such as a psychologist or psychiatrist. This typically involves a detailed interview to assess long-term patterns of behavior, thoughts, and emotions. The criteria for each disorder are outlined in the DSM-5, and a diagnosis is made if the individual's symptoms match the specific disorder's characteristics.
Diagnosis can be challenging because personality disorders often overlap with other mental health conditions, such as depression or anxiety. It's important for the mental health professional to rule out other possible causes and assess whether the symptoms are consistent across different situations and over a prolonged period.3. What Causes Personality Disorders? The exact causes of personality disorders are not fully understood, but research suggests that a combination of genetic, environmental, and psychological factors contributes to their development. Some key factors include: Genetics: Family studies suggest that personality disorders can run in families, indicating a possible genetic predisposition. However, no specific genes have been identified. Childhood Trauma: Adverse experiences during childhood, such as neglect, abuse, or unstable family relationships, are linked to an increased risk of developing a personality disorder. Brain Structure and Functioning: Some studies have found differences in the brain structure and functioning of individuals with certain personality disorders, particularly in areas related to emotional regulation and impulse control. While these factors may increase the likelihood of developing a personality disorder, not everyone with these risk factors will develop one.4. What Problems Can Be Linked to Personality Disorders? Personality disorders can lead to a wide range of issues that affect various aspects of a person's life. These may include: Relationship Difficulties: People with personality disorders often struggle with forming and maintaining stable relationships. Their behaviors and emotional patterns can cause conflicts and misunderstandings with others. Work and Social Problems: The inflexibility in thinking and behavior associated with personality disorders can lead to difficulties in the workplace and social settings. Individuals may have trouble adapting to changing situations or managing stress. Mental Health Issues: Personality disorders frequently co-occur with other mental health conditions, such as depression, anxiety, substance abuse, or eating disorders. Self-harm and Suicidal Behavior: Some personality disorders, particularly borderline personality disorder, are associated with an increased risk of self-harm and suicidal behavior, often as a way to cope with intense emotional pain. 5. When Wouldn’t a Hypnotherapist Work with a Client with a Personality Disorder? Hypnotherapy can be beneficial for many people, but working with clients who have personality disorders requires extra caution. In some cases, it may not be appropriate for a hypnotherapist to work with these clients at all, particularly if the client’s symptoms are severe, untreated, or if they are not under the care of a mental health professional. Here are some guidelines for hypnotherapists when considering working with clients who have personality disorders:Cluster A Disorders Clients with Cluster A disorders (paranoid, schizoid, schizotypal) may be less suited for hypnotherapy due to their suspicious or eccentric thought patterns, which can make it difficult for them to engage in the therapeutic process or trust the hypnotherapist. These individuals may be at risk of feeling manipulated or misinterpreting the therapist’s intentions.Extra Care with Clients in Cluster A: If a hypnotherapist does decide to work with someone in this group, extra care should be taken in building rapport and ensuring clear communication. Hypnotherapists should also screen for any use of antipsychotic medication and consult with the client’s psychiatrist or mental health professional. Liaising with a supervisor is crucial to ensure that the hypnotherapy process is handled with the appropriate sensitivity and care.Cluster B Disorders Clients with Cluster B disorders, especially those with borderline or antisocial personality disorder, may present particular challenges for hypnotherapists. These individuals may exhibit impulsive, erratic, or manipulative behavior that can disrupt the therapeutic process. In cases where there is a history of violence, self-harm, or significant instability, hypnotherapy may not be advisable without the involvement of a multidisciplinary mental health team.Cluster C Disorders While clients with Cluster C disorders may be anxious or overly dependent, they are often more willing to engage in hypnotherapy. However, hypnotherapists should be aware of the potential for these clients to become overly reliant on therapy, and clear boundaries should be set.In conclusion, hypnotherapists must exercise caution and professional judgment when considering working with clients who have personality disorders. A collaborative approach, including regular supervision and consultation with mental health professionals, is essential to ensuring that the client receives the appropriate care and support.

If you are a hypnotherapist and looking for support working with clients the Halifax Hypnotherapy Clinic Ltd offer 1:1 and group hypnotherapy supervision. Understanding Personality Disorders: A Hypnotherapists Guide

customer service survey feedback concept, businessman using smartphone give review and five stars rating to business services

As a clinical hypnotherapist, the topic of Google reviews or testimonials can feel like a bit of a double-edged sword. On one hand, positive reviews can significantly boost your visibility and credibility. On the other hand, when working with vulnerable clients, there are ethical, legal, and even personal considerations that make actively pushing for reviews a questionable practice. In this blog, I’ll explore some of the complexities around asking for reviews, particularly for professionals in the hypnotherapy field. The Legal Landscape: GDPR, Client Consent, and Insurance One of the first things to consider when asking clients for reviews or testimonials is the legal framework. The General Data Protection Regulation (GDPR) requires explicit consent for any personal data to be shared publicly. When a client leaves a review, especially if they mention details about their mental health or therapeutic process, that information is classified as sensitive data under GDPR. This means clients must fully understand how their data will be used, and they must give clear, informed consent, this needs to be part of your client agreement, wrote into your GDPR or privacy policy and also confirmed by your insurance company, to ensure it complies with informed consent, but it is not always that simple. A client may feel hesitant to say no to a therapist with whom they’ve built a trusting relationship. This could blur the lines of informed consent, as the client might feel pressured (even if unintentionally) due to the dynamics of the therapeutic relationship. As hypnotherapists, we are in positions of authority and trust, which can complicate the situation.

Client Vulnerability and Ethical Considerations For many clients, especially those experiencing anxiety, PTSD, or other mental health issues, the therapeutic space is a vulnerable one. Requesting a review could be uncomfortable or even distressing for some clients. For example, a client from a faith background may feel reluctant to share their experiences with hypnotherapy publicly due to stigma associated with therapy in their community or religion. Similarly, vulnerable clients who are experiencing severe anxiety or depression may not have the emotional bandwidth to assess whether or not leaving a review is truly in their best interest.Another concern is how a review might impact a client in the long term. A client who has overcome anxiety might leave a glowing review, thanking hypnotherapy for their transformation. However, months or years later, that same client may apply for a job, a mortgage, or even holiday insurance, forgetting that their review mentioned mental health issues. If these issues haven’t been declared in their applications, the review could complicate their situation—something they may not have considered at the time.Testimonials, the ASA, and CAP There’s also the issue of advertising standards. The Advertising Standards Authority (ASA) and the Committee of Advertising Practice (CAP) set guidelines for healthcare professionals, including hypnotherapists, when it comes to using testimonials. Testimonials cannot make medical claims or imply guaranteed outcomes, which can sometimes be difficult to control. A well-meaning client may leave a review saying they were "cured" of their anxiety or that hypnotherapy "completely healed" their PTSD. While these are positive sentiments, they could inadvertently lead to breaches of advertising regulations, potentially leaving the hypnotherapist in a tricky situation.

The Therapeutic Relationship: Implied Obligation The power imbalance inherent in the therapist-client relationship can also play a role in reviews. Clients may feel an unspoken pressure to leave a review, especially if they feel a sense of gratitude towards their therapist. For example, if a client has had a particularly positive experience overcoming a phobia, they might feel obliged to share this online, even if they are uncomfortable with public exposure. This can be particularly problematic for vulnerable clients, who may find it difficult to refuse such a request, even if the therapist doesn't explicitly ask for a review.Weighing the Pros and Cons From a personal perspective, I would never "encourage" reviews from clients dealing with anxiety or mental health-related issues. The ethical risks and potential complications are simply too high. Of course they can voluntarily leave a review via google or other platform and reviews can have a positive impact for services such as stop smoking hypnosis or help with phobias, where the therapeutic relationship may feel less vulnerable or personal. Even then, I would proceed with great care. Ultimately, reviews can be a helpful tool for building credibility, but for clinical hypnotherapists, we must tread carefully. Asking for reviews should never interfere with the client’s wellbeing, therapeutic process, or their longterm privacy. If you wouldn’t expect your doctor to ask for a review after a sensitive consultation, in my opinion we should hold ourselves to the same standard in clinical hypnotherapy.
<img id="nkaf85pizym23bxmk6" src="https://cdn.durable.co/getty/3bhm2N8PHu4zX2Pjm7xywYRPYjO6qOCGach7ymfdytYAxujPBxKGF3MZcJEcG0OA.jpeg" alt="Close-up of a male doctor hand hold a silver pen and showing pad in hospital.
